On an annual basis, by October 1st, the Office of Title VI submits a Compliance Plan and Implementation Manual to the Tennessee Human Rights Commission. The purpose of the report is to describe the long range goals and objectives that direct the Department of Health's efforts in the implementation and compliance of the Title VI Section of the 1964 Civil Rights Act.

The report has three sections: (1) the overview of the agency, (2) the approach to major civil rights functions, (3) the goals and objectives.

Finally each report is the summation of the combined efforts of the Department's Title VI Coordinating Committee which is comprised of representatives from within the department as well as the local community.

The Tennessee Department of Health does not tolerate harassment and discrimination based upon any protected class including race, color, national origin, sex, age, disability or reprisal or retaliation, in any program or activity conducted or funded by TDH. Such harassment and discrimination constitutes misconduct which undermines the integrity of the employment relationship and is subject to disciplinary action, up to and including dismissal.

Title VI Forms

In consideration of and for the purpose of obtaining Federal grants, loans, contracts, property, discounts or other Federal financial assistance from the Tennessee Department of Health the Applicant must provide these assurances:

  • Assurance of compliance with Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973.

  • Title IX of the Education Amendments of 1972 and the Age Discrimination Act of 1975.

As stated above, the Tennessee Department of Health prohibits discrimination against persons based on their race, color, national origin, sex, age, or disability. Any person or representative alleging discrimination based on a prohibited basis has the right to file a complaint within 180 days of the alleged discriminatory action with the Tennessee Department of Health.

2019-2020 Title VI Training

Title VI of the Civil Rights Act of 1964 is a national law that protects persons from discrimination based on their race, color, or national origin in programs and activities that receive federal financial assistance. In order for the Tennessee Department of Health to maintain compliance, a concentrated effort must be made to educate and train all departmental employees annually.

Click on the link below (register if you are new to the LifePath site, then go back to the sign-in page), sign in and click on the Sub-recipient Training module. After viewing the slides, go back to the “home” page and click on the – Test. After passing the test, go back to the “home” page and click on print Certificate. The Certificate will automatically print your name, the date, etc.; send the Certificate back with your signed contract.

All Sub-recipients are to take the “Sub-recipient training module".
