Food Retail Plan Review

Please note the requirements needed when seeking to (a) construct a new, (b) change, or (c) acquire an existing retail food establishment.

If the retail business is taking on a new name or undergoing renovation, please complete the Food Retail / Plan Review Questionnaire, returning it to Consumer and Industry Services (CIS) along with:

• Floor plan of food establishment showing the location of equipment, plumbing, electrical services, mechanical ventilation, and which is to include the three compartment sink, hand sink and mop sink.
• Labeling of all products produced and packaged in the establishment as is applicable.
• Business license or registration with the Tennessee Department of Revenue.
• A lease agreement that is signed, dated and notarized.
• A certificate of occupancy which is issued by a municipal or county building department.

If the business is keeping the acquired business' original name, complete the Food Retail / Plan Review Questionnaire and return it to CIS along with:

• Business license or registration with the Tennessee Department of Revenue.
• A lease agreement that is signed, dated and notarized.

The completed questionnaire and required documentation can be sent as email attachments to NewFood.Business@TN.gov or mailed to
