Follow these steps.
1. Department of Human Services (DHS)
- In our state, local DHS offices take applications for TennCare and other programs. All 95 counties have a DHS office.
- Find out the address and phone number of your local DHS office.
2. Getting an Application
- You don’t need an appointment to receive an application. If you have one, you may not have to wait as long. To get an application, you can either:
3. Giving the Application to DHS
- If you can, have an application filled out to take with you to DHS. Take it when you have your appointment with your DHS worker.
4. Disabilities
- DHS also makes disability determinations. This is done for the Social Security Administration (SSA). These persons may also be eligible for TennCare. To find out more, call 1-800-342-1117.

What if you are not eligible for Medicaid? You could still be eligible for TennCare Standard. Your local DHS worker can tell you.
