Self Determination
Waiver Program

The Tennessee Self Determination Waiver Program (#0427.01), as approved July 1, 2006, provides community-based services to individuals who would otherwise require the level of care provided in an Intermediate Care Facility (ICF/MR).

This program allows the individual to self Direct services, including services such as personal assistance.

The Division of Mental Retardation Services (DMRS) in the Tennessee Department of Finance and Administration serves as the Operational Administrative Agency for this waiver, which is administered under the supervision of the Bureau of TennCare.

To enroll in this HCBS waiver program, you must:

  • Live in Tennessee;
  • Have mental retardation before age 18 OR have a developmental disbility and be age 4 or younger;
  • Be financially eligible for Medicaid;
  • Meet TennCare criteria for payment of institutional ICF/MR care;
  • Be on the DMRS Waiting List and be classified in one of the crisis, urgent, or active waiting list categories prioritized by urgency of need;
  • Have an adequate support system to assure health and safety while receiving services in a home and community based setting (i.e., caregiver, personal emergency response system); and
  • Not need staff-supported residential services.

If you qualify for care in an ICF/MR, you will have the right to choose between care in the home or institutional ICF/MR care. If you choose to receive HCBS waiver program services in the home, you will still be eligible to receive your regular TennCare benefits. In addition, you will also be able to receive the following services if needed:

  • Behavioral Respite Services
  • Behavior Services
  • Day Services Respite
  • Environmental Accessibility Modifications
  • Financial Administration
  • Individual Transportation Services
  • Nutrition Services
  • Nursing Services
  • Occupational Therapy Services
  • Orientation and Mobility Training
  • Personal Assistance
  • Personal Emergency Response Systems
  • Physical Therapy Services
  • Respite
  • Specialized Medical Equipment (Supplies, & Assistive Technology)
  • Speech, Language, & Hearing Services
  • Vehicle Accessibility Modifications
