  1. You must file your claim by phone by contacting the UI Claims Center if you live in one of the following counties. If you do not live in one of the counties listed above, you file your claim in the local unemployment office closest to your home. You should bring your original DD form 214, member #4, and two forms of identification, including your social security card. If you have not received your DD form 214, you may still report to file a claim.
