ADA DECISION NOTICES TO CLIENTS

When a client requests an accommodation or
the Department or contractor recognizes that an accommodation is needed, the
client must be notified that the request has been considered and given the
details of the outcome by the caseworker, client rep or contractor to whom
the accommodation request was made.
The notice must include: ·
The
accommodation requested. ·
Whether the
request was approved or denied. ·
The reason
for the denial. ·
Any
alternative accommodations offered. ·
The
Compliance Officer’s address. ·
Instructions
on the steps needed to make a complaint to the Compliance Officer. |
