POLICY STATEMENT |

Make
applications available to individuals who request an application in person. ·
Mail the
application on the same day an application request is received by mail,
phone, FAX, or on-line. ·
The Families
First application process begins with a signed request for assistance on the
Department’s Family Assistance application form HS-0169. ·
DHS will
accept signed and dated applications received in the county office. The application may be received in person,
by mail, by fax, or on-line. The
application file date will be the date the application is received in the
county office, either by fax, mail, person, or on-line. The file date for a faxed application
includes holidays and weekends. Faxed
applications that contain the applicant’s name, address, and handwritten
signature are acceptable. Receipt of
an original application in addition to the fax is not required. ·
The
application can be used for all Family
Assistance programs. The applicant
indicates the particular program for which he/she wishes to apply. |
