GOOD CAUSE |

Good Cause for a delay in obtaining
immunizations or checkups will be granted for the reasons listed below: ·
The child is
ill and the health care provider recommends delaying immunization until the
child is well. ·
A health
care provider does not respond to requests for information to document
immunization or checkups. ·
A caretaker
has made a good faith effort to schedule checkups or immunizations within one
month, but has not been able to do so because of circumstances beyond the
caretaker’s control. ·
There are
other extenuating circumstances that could delay immunization or checkups. |
