ACCEPTABLE
SOURCES FOR VERIFICATION OF INCOME

Paycheck
stubs covering a minimum of the last four (4) weeks of regular income. A determination should be made whether or
not it is representative of usual pay.
The most recent stub submitted should be no more than 30 days old. ·
Copies of
court orders or legal documents ·
Records of
county or circuit courts ·
Award
letters, approval of benefits notices, written statements, both signed and
dated, of employers showing weekly number of work hours, rate of pay, and
gross income (when check stub or payment histories are not available) ·
W-2 Forms or
income tax forms, i.e., Form C ·
Bank or financial
institution records ·
Business
records maintained by self-employed persons ·
Electronic
