IMPORTANT
HSDA staff has made some proposed revisions to the CON Application Form. The purpose of these revisions include reducing duplication in the questions, enhancing uniformity in reporting, reducing supplemental questions, making terminology changes, and making other minor edits. The areas in the draft application form that are proposed for change are highlighted in yellow. New text is in red and text to be deleted has a strikethrough.
Notification will be provided well in advance of the implementation of the revised CON applications form.
