Answer the legal questions, sign, and date the application. Check the Address Change Notification box at the bottom of the application. Mail application to the address provided at the top of the application.

You may check our Educator License Information web site to verify receipt of licensure information. Unless you already know your license number we suggest first using the public search option to locate your license number by only entering your first and last name as listed on the application. Click on the name, the license information will be revealed. If you already hold a license and know your license number you may use the Educator Log in option.

 

In an effort to deliver improved customer service and reduced costs, the Office of Teacher Licensing will no longer print paper licenses for educators on July 1. This change does not impact the way an educator applies for licensing or the way LEAs verify licenses. By using the public search option of the Online Licensing Application you will be able to obtain a printer friendly version for your use. (Options of legal verification accepted by the Office of Teacher Licensing are: a photocopy of the notarized or certified marriage license, divorce decree, or the court order that generated the legal change of name. Social security cards or drivers licenses are not acceptable forms of legal verification of name change). Mail application to the address provided at the top of the application.
