Producers - Retaining Ownership (QSA)

 

1.   Producer must submit the following documentation to the VIP Coordinator:
      a.  Producer Information Sheet
      b.  Pre-Conditioning Form for group of cattle producer is marketing
           • Under EID numbers please list feedlot tag ID #’s
      c.  Documentation from a USDA Age and Source Program verifying age and source of cattle
           (example: PVP Certificate or QSA Form)
      d.  Copy of initial yard report

2.  VIP Coordinator will then mail the producer
     a.  Grant Authorization and Payment Form
     b.  Substitute W-9 to be completed and signed

3.  Producer must return forms back to the VIP Coordinator for payment to be issued
